Mama & Milk is in-network with United Healthcare, UMR, Aetna, BCBS of TN, Anthem and serving Cigna clients through Wildflower Health.
How to schedule a visit: While many families find their visits are covered, I always encourage you to contact your insurance provider before booking so you are aware of your lactation benefits and any potential financial responsibility. Additional information specific to each insurance plan is available on its page-simply click the box for your insurance provider and complete the form. After submission, I contact you to schedule your visit. In most cases, you will hear from me the same day you submit your information. If you need an urgent appointment or have any questions, please call or text me at 423-401-0705.
If you do not see your insurance plan listed above, I recommend contacting your insurance provider directly by calling the number on your insurance card to verify the number of lactation visits covered under your plan. In this case, payment to Mama & Milk will be required upfront; however, I will provide you with a superbill that you can submit to your insurance company for potential reimbursement.

I provide all self pay clients with a superbill to pursue reimbursement for our visit from their insurance company. I accept cash, debit, credit, and HSA cards.
